We are seeking compassionate and motivated individuals; professional care experience not essential.
Home First is the county wide Urgent Care and Reablement Service managed by Herefordshire Council. We support people in the community to regain maximum independence following an admission to hospital, a crisis at home, illness or disability, or any change in circumstances that has impacted their independence.
Our aim is to give individuals the skills, ability and knowledge to live their everyday life as independently as possible, and we do this through setting and working towards goals for a period of up to 6 weeks.

What is Reablement?
Reablement is the official title given to short-term care at home and Home First is available to support people who may have experienced an admission to hospital, a crisis at home, a temporary illness or permanent disability. A Reablement Worker is there to support the individual and help them get back on their feet.
How would you support people as a Reablement Worker?
Help individuals regain skills and confidence in completing everyday tasks and activities such as making meals, washing and dressing.
Support individuals to gain new skills to help them manage at home.
Help avoid unnecessary admission to hospital, or long-term residential care.
Help individuals to get back on their feet after a stay in hospital.
The tasks you may be involved in -
Personal hygiene: Enabling an individual to regain their ability to wash and dress themselves, return to showering or bathing
Domestic tasks: Enabling an individual to prepare/cook meals for themselves
Medication support: Administering medication, application of creams, always identifying ways the individual can do this themselves.
Welfare – just checking that individuals are safe and well
Moving and handling – e.g. using correct equipment to assist with all required transfers (i.e. getting out of bed/ chair/shower).
Problem solving with individuals to do tasks differently that enables them to do to the tasks by themselves.
